COVID-19 Coronavirus Precautions for the Employee Health Clinic

EMPLOYEES & PATIENTS:
• Regardless, of your healthcare needs, pre-existing clinic procedures 
  and managerial directives, call first!
• You MUST call the clinic first, do not just show up at the clinic. 
• Do not enter the clinic building without an appointment.
The clinic will triage your symptoms over the phone and make a determination, if the 
employee health clinic is the appropriate place for you to receive treatment. 
 

If you have any of these symptoms:
Fever
Cough
Sore Throat 
Shortness of breath

AND

You have traveled to (including transit through) mainland China, Hong Kong, Indonesia, Iran, Italy, Japan, Singapore, South Korea, or Thailand in the 14 days before getting sick (or any other regions of High Risk) including cruise ships

OR

If you have had close contact with someone diagnosed with, or suspected of having 
COVID-19 cornavirus in the 14 days before getting sick.

CALL the local Florida Department of Health in Polk for direction, (866) 779-6121. If you are concerned, but unsure of your symptoms, you may CALL the employee health clinic for direction (863) 534-5280. Again, DO NOT visit the clinic without an appointment or 
calling first.

Note: Failure to follow these instructions could result in a quarantine, which would lead 
            to the shutdown of the entire clinic.
